Case presentation: A 25 year old female patient presented to our restorative department
Patient’s chief complain was pain in the lower right posterior area, after clinical and radiographic examination the following was diagnosed:
Deeply carious LR 6 and 7 with enamel
Case difficulty:
1. Enlarged Gingiva the made Isolation very tricky and difficult
2. Very deep caries and cracks were found which forced me to open marginal ridges and made the case more complicated
3. Anatomical complexity to be reproduced with well defined cusp ridges and occlusal topography
4. A small limitation of mouth opening
Treatment
1. IANB was administrated to the patient
2. Amalgam removal under rubber dam Isolation
3. Rubber dam isolation was changed to ensure aseptic and clean field avoiding any bacterial or metal contamination during caries management
4. Deep caries management was done in LR 6 and 7 by the selective caries removal approach to avoid any pulp and obtaining a peripheral seal zone
5. Selective enamel etching was done followed application of the universal adhesive, G-PREMIO bond
6. Proximal walls were built first and elevated margins using G-aenial universal injectable composite, dentin replacement was completed using Ever X flow maximizing the degree of reinforcement
7. Full cavity and morphological reconstruction was completed Using G-aenial universal injectable composite following the cusp by cusp technique, restoring all the anatomical details of the occlusal surface.
8. Finishing the restoration was completed using a yellow coded flame stone and finishing disks to smoothen the margins of the restoration and adjusting the occlusion
9. Final finishing and polishing using twist wheels and Polishing paste
