Case Report: Veneers for Generalized Hypocalcification
Patient Presentation:
- Female patient presenting with generalized hypocalcification of the upper arch.
- Chief complaint: unsatisfactory tooth shape, inconsistent shades, and compromised esthetics.
- Lower arch displayed only mild discoloration; no major intervention was required.
Treatment Steps:
- Initial Records & Assessment
- Provisional photographs were taken to document the initial condition.
- Comprehensive examination and diagnosis were performed.
- A detailed treatment plan was established.
- Smile Design
- Digital smile design was created using ExoCAD software.
- The design was translated into a 3D printed diagnostic cast.
- Mock-up & Preparation Guide
- A silicone index was fabricated from the 3D model.
- The index was filled with TempeCal material and transferred intraorally, producing a precise mock-up.
- A three-disc depth cutter bur (commonly known as the “three-wheel depth cutter”) was utilized to achieve accurate and controlled tooth reduction.
- Tooth Preparation
- Guided reduction was performed, following the mock-up and depth cuts.
- Reduction was uniform, symmetrical, and consistent with the proposed smile design.
- Shade Selection
- Shade analysis was carried out under proper lighting conditions.
- Selected shade harmonized with the lower arch, ensuring a natural and seamless esthetic outcome.
- Try-in Phase
- Multiple try-ins were performed to verify:
- Marginal integrity
- Proximal contacts
- Functional alignment
- Esthetic integration with facial proportions
- Final Delivery
- Veneers were bonded using adhesive protocols.
- Post-operative photographs were taken to document the transformation.
Outcome:
The veneers did more than mask defects; they restored balance, harmony, and radiance to the patient’s smile. What was once a source of insecurity became a symbol of confidence. Each ceramic shell was not only a restoration, but also a brushstroke in the patient’s portrait of self-assurance.
