Title
Endo-Restorative Rehabilitation of a Grossly Compromised Upper Molar Under Rubber Dam Isolation
Subtitle
A step-by-step biomimetic workflow combining endodontic treatment, adhesive foundation build-up, and cuspal coverage restoration
Introduction
Restoring a severely compromised posterior tooth requires more than endodontic treatment alone. The long-term prognosis depends on isolation, coronal seal, remaining tooth structure, and final cuspal protection. This case demonstrates a protocol-driven approach for managing a grossly carious upper molar with endodontic treatment followed by adhesive cuspal coverage restoration.
Case Overview
The patient presented with a grossly carious upper molar with extensive coronal breakdown and pulpal involvement. The tooth was considered restorable after clinical and radiographic assessment. The treatment plan focused on preserving the tooth through endodontic therapy and immediate adhesive reinforcement.
Step-by-Step Clinical Workflow
Step 1: Preoperative Assessment
Clinical examination and radiographs were taken to evaluate caries depth, pulpal involvement, root anatomy, and restorability. The tooth showed extensive structural loss but adequate remaining tooth structure for adhesive rehabilitation.
Step 2: Isolation
Rubber dam isolation was achieved to create a clean and controlled working field. Isolation was critical for both endodontic disinfection and predictable adhesive bonding.
Step 3: Caries Removal and Access Refinement
All infected dentin was removed while preserving sound tooth structure. The access cavity was refined carefully to improve visibility and locate canal orifices without unnecessary removal of dentin.
Step 4: Canal Identification and Negotiation
Canal orifices were identified under magnification. Patency and working length were established using hand files, apex locator, and radiographic confirmation.
Step 5: Cleaning and Shaping
The canals were shaped with controlled instrumentation while maintaining original canal anatomy. Copious irrigation was used throughout the procedure to support disinfection and debris removal.
Step 6: Irrigation Protocol
A structured irrigation protocol was followed using sodium hypochlorite, saline flush, EDTA, and final activation. This step is essential for smear layer removal and improved sealer adaptation.
Step 7: Obturation
The canals were obturated with a bioceramic-based approach to achieve an apical and coronal seal. Post-obturation radiographs confirmed satisfactory canal filling and length control.
Step 8: Adhesive Foundation Build-Up
After endodontic completion, the chamber was cleaned and prepared for bonding. Adhesive protocols were followed to seal the dentin and rebuild the missing tooth structure.
Step 9: Cuspal Coverage Restoration
Due to the extent of coronal destruction and weakened cusps, cuspal coverage was planned. The restoration was built to reinforce the remaining tooth structure, restore occlusal anatomy, and reduce fracture risk.
Step 10: Finishing, Polishing, and Occlusion
Final finishing and polishing were performed to achieve smooth margins and anatomical form. Occlusion was checked carefully to avoid heavy contacts on the restored cusps.
Discussion
Endodontically treated posterior teeth are more prone to fracture when marginal ridges and cuspal integrity are compromised. For this reason, the final restoration plays a major role in long-term success. In this case, rubber dam isolation, conservative access, controlled irrigation, and adhesive cuspal coverage allowed the tooth to be restored predictably while preserving maximum remaining structure.
Conclusion
A severely compromised molar can often be saved when endodontics and restorative dentistry are planned together. The key is not only cleaning and obturating the canals, but also rebuilding the tooth with a strong coronal seal and cuspal protection.
