Comprehensive Dental Rehabilitation
Reason for Visit: A desire to resolve all dental issues comprehensively and permanently. Serhii came to our clinic Sergatiy Dental Clinic seeking a complete transformation of his oral health and smile aesthetics, understanding that a thorough, multidisciplinary approach would be necessary to achieve lasting results.
Initial Condition:
- Multiple carious lesions affecting nearly all teeth, with varying degrees of severity
- Significant tooth wear caused by bruxism and improper occlusion
- Malocclusion affecting both function and aesthetics
- Missing tooth 4.6 in the posterior mandibular region, leading to altered bite dynamics
- Compromised smile aesthetics with discoloration and irregular tooth alignment

Treatment Stages
Stage 1: Sanation & Preparatory Treatment
Periodontal therapy and comprehensive caries treatment. All carious lesions were treated using modern composite materials, ensuring proper tooth structure preservation and preparing a healthy foundation for subsequent procedures.
Preparation for orthodontic treatment. This included professional hygiene, oral health education, and establishing optimal conditions for bracket placement and orthodontic mechanics.

Stage 2: Surgical Phase
Extraction of third molars (wisdom teeth). The removal of impacted wisdom teeth was necessary to prevent future complications and to create adequate space for proper orthodontic tooth movement and alignment.

Stage 3: Smile Prototype Try-In
Transfer of the proposed smile design into the mouth using a non-invasive mock-up, providing motivation and visualization of the expected treatment outcome before tooth preparation. This critical step allowed Serhii to preview his future smile and provided valuable feedback for fine-tuning the aesthetic parameters. The mock-up was created based on digital smile design principles, considering facial proportions, lip dynamics, and personal preferences.
Stage 4: Orthodontic Treatment
Orthodontic therapy with metal brackets on both arches, using orthodontic mini-implants (TADs) for controlled tooth movement. The treatment duration was approximately 18 months, during which we achieved proper tooth alignment, corrected the occlusal plane, and established ideal spacing for future restorations. Temporary anchorage devices enabled precise biomechanical control, allowing us to move teeth in directions that would be difficult or impossible with conventional orthodontics alone.

Stage 5: Aesthetic & Functional Rehabilitation
Axiographic analysis to record and analyze jaw movements, ensuring that final restorations would be in harmony with the patient’s unique temporomandibular joint dynamics.
Splint therapy to deprogram muscle patterns, relieve TMJ tension, and establish a physiologically optimal jaw position before final restorations.
Implant placement in site 4.6 using a premium implant system with immediate torque stability, followed by a controlled osseointegration period.
Functional wax-up created in the dental laboratory to simulate the final tooth morphology, occlusal contacts, and aesthetic appearance in three dimensions.
Tooth preparation and fabrication of 28 ceramic restorations (veneers, onlays, crowns). Each restoration was meticulously crafted from high-strength ceramic materials, combining exceptional aesthetics with long-term durability.
Final adhesive bonding of ceramic restorations in both arches using state-of-the-art bonding protocols, ensuring maximum retention and marginal seal for lasting results.
The final rehabilitation phase integrated all previous treatment stages into a cohesive result. Special attention was paid to recreating natural tooth translucency, surface texture, and color gradients to achieve a smile that appears both beautiful and authentic.
