-I will not talk about the details of the protocol of doing the restoration in tooth 16 (the first molar)
– I want to bring your attention to the open contact between teeth 15 and 14 (the premolars) that had been closed during the procedure.
– Some of the points are based on my clinical seen.
-In this case the patient has a new amalgam restoration ( done by other dentist) that he didn’t like and wanted to replace it with a new composite restoration. There is a deep proximal caries mesially in tooth 16.
– The separation force of the Bioclear ring which is made of 2 NiTi wire is very high and more over if it was with active wedge. the patient may feel it like his teeth had been separated or moved when applied. And the clinician will feel its force when he try to open it and when he try to apply.
-In this case the separation force was high enough to move the tooth 15 about 1 mm ( less or more) immediately with continuous pressure in the contact area that was enough to shift the tooth 15 mesially and close the contact with the adjacent 14.
– After three days the tooth 15 shifted completely to close the contact with the 14. And after 12 day there was even a healthy papilla that filled the triangle and was satisfied for the patient.
