Clinical presentation:
Middle aged female pt presented with a lower second molar that has a large class V involving occlusal surface..on applying air instant response was observed that disappears after few seconds with no signs of irreversible pulpitis.
Hence, peripheral seal zone was targeted
3 challenges were included:
1- How to obtain buccal margin (after caries removal as it’s almost subgingival)?
A: First, an impregnated retraction cord was introduced into the gingival sulcus.
Then, teflon tape above retraction cord for a dry field as much as possible, God willing.
2- How to build up class V (while application of different matrices failed as the clamped molar is the operated molar)?
A: Injection molding tech. , a composite compule by Dentsply was used, injected composite was condensed to obtain a well adapted basic layer. This obtained a flat buccal profile.
3- Here comes the final challenge; how to regain the buccal contour?
A: Packable composite by Shofu was used, successive build up technique to overcome c-factor.
