ABSTRACT

This case demonstrates the use of microscope-assisted direct composite veneers to restore aesthetics, form, and harmony in the anterior region with a fully guided digital 3D Exocad mock-up. The combination of digital planning, microscope precision, and high-definition photographic documentation enabled a controlled execution of biomimetic layering using 3M Easy Match, Tokuyama Omega, Saremco Flowables, and SDI tints. Finishing and polishing were achieved using a multi-step protocol with Dentsply Enhance, Kerr Polishers, Lucida (Style Italiano), and Diashine for lifelike surface texture and gloss.

CASE OVERVIEW

A young adult patient presented seeking a natural smile improvement for upper anterior teeth (upper 4), expressing dissatisfaction with discolouration and inter-tooth spacing. The treatment objective was to design and execute a non-invasive, additive composite veneer rehabilitation preserving all enamel and soft-tissue architecture.

DIGITAL DESIGN & MOCK-UP

A full photographic series was captured using a Sony Alpha 7R V mirrorless system with Godox MF-12 macro diffusers and Intra.Diffuser SMD lighting to ensure consistent illumination, colour accuracy, and shade mapping. The images were imported into Exocad for a 3D digital smile simulation, aligning facial midline, incisal curvature, and tooth proportions.

The mock-up served as both a communication and clinical transfer tool. The digital plan guided composite stratification to achieve symmetry and proportion consistent with facial dynamics.

CLINICAL PROCEDURE

Isolation & Preparation

Full rubber dam isolation was applied for moisture control and field stability. No enamel reduction was performed; only minimal surface cleaning and micro-etching were used to enhance micromechanical retention.

Adhesive Protocol

Bisco Universal Bond was applied in multiple coats and gently air-thinned to optimise dentine and enamel infiltration.

Composite Layering under Microscope

Working under ×10–×16 magnification ensured precise anatomic sculpting and margin blending.

Palatal enamel wall established using a transparent matrix guided by the Exocad mock-up.
Dentine core: built with 3M Easy Match dentine body to replicate chroma and inner opacity.
Transition zone: controlled with Saremco Flowable A2 for intermediate translucency.
Enamel halo: enhanced using Tokuyama Omega translucent shade.
Micro characterisation: achieved with SDI stains (white and grey) to mimic perikymata and incisal depth.

Each increment was light-cured through the microscope to confirm thickness control and marginal adaptation.

FINISHING & POLISHING SEQUENCE

Finishing was performed using a four-stage system:

Dentsply Enhance Finishing Discs for contour refinement.
Kerr Polishers for surface pre-gloss.
Lucida (Style Italiano) diamond polishers for high-gloss morphology.
Diashine Paste for final enamel-like lustre.

The microscope aided in visualising macro- and micro-texture consistency across surfaces. Final gloss replicated natural enamel reflectivity and halo phenomena.

RESULT & EVALUATION

The outcome achieved a natural youthful smile with balanced midline, incisal embrasures, and value harmony. Surface texture and light diffusion closely matched adjacent dentition. The patient expressed high satisfaction with both function and aesthetics.

DISCUSSION

This case highlights how digital smile design, magnification, and biomimetic composite layering integrate to yield a predictable, additive, and reversible aesthetic rehabilitation. The Exocad mock-up served as the architectural blueprint, while microscope precision and high-fidelity photography ensured traceable execution and documentation.

CONCLUSION

Microscope-assisted composite veneers performed within a digital smile workflow demonstrate that modern adhesion, controlled morphology, and surface micro-texture can achieve results comparable to ceramics while preserving natural tooth integrity. The combination of planning, isolation, and optical layering defines the biomimetic approach for contemporary minimally invasive aesthetics.
