Introduction
Subgingival margins in grossly carious posterior teeth complicate isolation and compromise both restorative and endodontic outcomes. Deep Margin Elevation (DME) provides a predictable solution by relocating margins coronally, improving access and sealing ability.
This case demonstrates pre-endodontic DME in a mandibular second molar with a C-shaped canal system, emphasizing the importance of restoring the foundation before initiating endodontic therapy.
Diagnosis & Treatment Plan
- Mandibular second molar (7)
- Gross proximal caries extending subgingivally
- Suspected/confirmed C-shaped canal configuration
Plan:
- Rubber dam isolation
- Caries removal and peripheral seal establishment
- Class II build-up with Deep Margin Elevation (DME)
- Proceed with endodontic treatment
Clinical Procedure
Step 1 – Isolation
- Rubber dam placement with proper inversion
- Critical due to subgingival margins
Step 2 – Caries Removal
- Complete cleaning of peripheral seal zone
- Controlled excavation near deep areas
Step 3 – Matrix & Adaptation
- Sectional matrix system placed
- Careful wedging to seal gingival margin
- Matrix stability verified before bonding
Step 4 – Adhesive Protocol
- Selective enamel etch
- Application of bonding system
- Proper curing to ensure marginal integrity
Step 5 – Deep Margin Elevation (DME)
- Composite placed to elevate margin coronally
- Gingival margin converted to a supragingival/restorable level
- Ensured smooth transition and seal
Step 6 – Pre-Endo Build-Up
- Proximal wall completed
- Access cavity now possible in a clean, controlled field
Key Clinical Points
- DME is essential when margins compromise isolation
- A clean peripheral seal zone defines long-term success
- Pre-Endo build-up improves:
- Isolation
- Visibility
- Instrument control
- C-shaped canals demand better access and irrigation control
- Foundation first → Endo second
Outcome
- Margin successfully elevated and sealed
- Isolation achieved predictably
- Tooth prepared for controlled endodontic therapy
- Structural integrity improved prior to instrumentation
Conclusion
Pre-endodontic DME transforms an otherwise compromised case into a predictable one. In complex anatomies like C-shaped molars, restoring the coronal structure first significantly improves endodontic outcomes.
