Direct composite overlays can provide a conservative restorative option for posterior teeth with extensive structural loss while preserving the remaining sound tooth structure.
In this case, the tooth was isolated using a rubber dam, followed by conservative removal of compromised tooth structure. A direct adhesive composite technique was then used to rebuild the missing cuspal and occlusal anatomy.
The restoration was designed to provide adequate cuspal coverage, functional anatomy, and occlusal stability while maintaining a minimally invasive approach.
This case highlights the potential of direct composite overlays as a cost-effective and conservative alternative to indirect restorations in appropriately selected posterior cases.
