We report the case of a 50-year-old female patient presenting with an odontogenic cyst associated with tooth 35. Surgical management was carried out in two stages. Initially, cyst enucleation and apicoectomy were performed, as it was not possible to adequately dry the root canal to allow for orthograde therapy. Subsequently, an orthograde root canal treatment was completed once favorable conditions were achieved.
