22 Female patient came to our clinic seeking for a new smile and a solution for her gummy problems, A 2 months treatment that will change her story and how she smiles.
we asked her for the problems she is facing, and if she had previous treatment, she said that she didn’t had any treatment for her gum
we had to make our first visit to talk only and take impressions and some pictures and videos to the record
we explained to her that the treatment will take time because she had soft tissue problems and will take time to heal and stabilize.
We start by make a mock-up to see what are her exceptions and what result she expect.
lets see the procedure
The before picture
she obviously don't brush her teeth, all her anteriors are square in shape due to the gum problems she have.
The teeth need treatment and the gum need to be healthy and correct
she don't have shifting in midline
the smile line and papilla line
we measure the length of the right central to calibrate the ruler in Keynote
The design of the teeth (DSD planing)
we will need 2.5mm to be removed from the gum starting from the incisor and keep on to other teeth
we have to make bone sounding (measure the distance from the free gingival margin to the bone level) to see how much we can remove
everything look perfect, we need to remove 2.5mm from the gum and we had 5 mm to the bone crest, so we can remove 2.5mm and then new gingival margin will be 2.5mm to the bone crest and that's not a problem since she had medium thick biotype.
no need for bone removal
3D printed Gingivectomy guide according to our DSD planing
2.5mm Exactly
Now all her teeth in their actual size
the removal of gum done under local anesthesia with diode laser
The difference between the teeth size before and after gingivectom.
we will leave the gum for 3 weeks to stabilize
her new smile with new gum level
