Anterior Composite Veneers for Fractured Central Incisors – A Minimally Invasive Approach

This case demonstrates the restoration of fractured maxillary central incisors and the correction of esthetics using direct composite veneers under strict isolation. The workflow highlights the importance of adhesion, proper field control, and minimally invasive dentistry to achieve natural esthetics and functional harmony.

Clinical Workflow

1. Initial Presentation

The patient presented with fractured anterior teeth (non-complicated crown fracture) sustained 3 years ago. Cervical staining and compromised esthetics were also noted. After evaluation, a treatment plan involving direct composite veneers was selected to restore natural esthetics and function.

2. Rubber Dam Isolation

Complete isolation with a rubber dam and clamps ensured:

Moisture control
Improved visibility
Predictable adhesion and layering

3. Prophylaxis

Stains were carefully removed, exposing sound enamel and dentin. Minimal preparation was performed to preserve maximum healthy structure.

4. Tooth Preparation

Beveling of enamel margins
Surface finishing to enhance bonding
Preparation designed to allow esthetic blending of composite margins

5. Matrices & Silicone Index

Mylar strip and Bioclear transparent matrices were used for proximal contouring.
Silicone index (Memosil 2, Kulzer – clear putty wash) guided palatal contours and symmetry.

6. Composite Build-Up

A layering technique was performed using nanohybrid composites for natural esthetics:

Dentin Layer: Warm shade (3M Easy Match)
Enamel Layer: Natural shade (3M Easy Match)
Body Layer: Clearfil Majesty ES2 (Kuraray)
Characterization:
- Incisal halo effect with SDI blue/lavender stain
- White stain by Tokuyama
- Border definition with Dentsply ST Effect (E1 shade)

7. Finishing & Polishing

Sequential finishing and polishing systems were employed to recreate natural surface texture and gloss:

Pre-polishing: Pascal / Diashine kit
Finishing: Dentsply Enhance & PoGo system
Final Polishing: Lucida system (StyleItaliano) with star wheels

8. Final Result

The restorations blended seamlessly with the natural dentition, restoring:

Functional harmony
Long-term predictability
