34 YO female pt. came with sever pain in upper right premolars area after clinical examination and evaluation enhanced by radiograph findings we found badly destructed UR4 involving pulp causing irreversible pulpitis with gingival over growth which is very painful during bite and found carious lesion in UR5 involving enamel and dentin in mesial aspect causing reversible pulpitis
TREATMENT PROTOCOL :
- LA for MSAN branches
- Caries removal and gingivectomy
- CDD to ensure copleted caries removal
- Margin exposure
- multiple rubber dam isolation
- DME on 2 steps and pre-endo build up for 4
- RCT for 4 after strect irrigation protocol
- Using single cone with BC sealer in OBTURATION
- Class II cavity design for 5 (BACK TO BACK)
- Direct composite overlay for 4 and direct composite resto for 5
- Using SNOW BLOW TECH. for DME and class II composite build up
