Patient:
35-year-old patient presented with missed upper right posterior teeth ( First and second premolars )
Diagnosis:
Missed upper right posterior teeth, First and second premolars with simple caries in occlusal surface of upper right first molar.
Treatment Plan:
_ Remove caries of upper right first molar and restore it with composite restoration in the same visit of our preparation of abutment teeth.
_ Preparation of our abutment teeth ( Upper right canine and upper right first molar ), with chamfer finch line 1mm.
_Our prosthodontic restoration is PFM.
Procedure:
– Caries removal of upper right first molar under rubber dam.
– Etching and bonding.
– Restore our cavity by using composite.
– Preparation of our abutment teeth with tapered diamond stone with round end 1mm under coolant for making Supra gingival finish line.
– Finishing and polishing prepared abutment teeth.
Materials:
– Composite: (DX universal)
– Adhesive: (Universal, seventh generation)
– Burs: 1.. Tapered diamond stone with round end 1mm ( coarse blue ) for preparation upper first molar and labial surface of upper canine.
2.. Flamed diamond stone for preparation of palatal surface of upper canine ( coarse blue )
3.. Diamond stone finishing bur ( fine yellow )
4.. Retraction cord ( twisted ) size 1.
5.. Condensation silicone for impression of upper arch.
6.. Alginate impression material for impression of opposite lower arch.
7.. Pink sheet wax for bite registration.
8.. GIC for cementation.
Outcome:
Proper caries removal and restoration, precise restoration of upper missed premolars with PFM restorations.
Conclusion:
Supra gingival finish line can simplify all steps of our prosthodontic procedure ( Preparation, application of retraction cord, impression making, finishing and polishing, proper removal of excess cement and easy cleaning of our restoration ) .
