Class II Direct Composite Restorations with Sectional Matrix System – A Conservative Approach

This case demonstrates the management of proximal caries in posterior teeth using direct composite restorations with proper isolation and sectional matrix technique. The goal was to achieve functional occlusion, tight proximal contacts, and seamless esthetics through minimally invasive adhesive dentistry.

Clinical Workflow

1. Initial Presentation

The patient presented with carious lesions in mandibular molars and premolars.
Radiographic examination confirmed proximal caries without pulpal involvement.
A conservative adhesive restorative plan with direct composite fillings was chosen.

2. Rubber Dam Isolation

Complete isolation was achieved using a rubber dam and clamps.
This ensured a moisture-free environment, critical for predictable bonding and caries excavation.

3. Caries Removal & Cavity Preparation

Carious dentin was selectively removed.
Conservative Class II cavity designs were created to preserve tooth structure.
Gingival seat and proximal box preparation were refined to receive the restoration.

4. Matrix & Wedge Placement

Sectional matrix system (Garrisons) with separation rings was placed.
Wooden wedges ensured gingival seal and proper proximal contour.
This allowed for optimal emergence profile and tight contact area reconstruction.

5. Adhesive Protocol

Selective enamel etching with phosphoric acid was performed.
Universal bonding agent (Clearfil SE Bond applied under rubber dam isolation.

6. Composite Build-Up

Incremental layering of nanohybrid composite was carried out:
- Dentin replacement layers for strength
- Enamel replacement layers for esthetics and translucency
Each increment was light-cured adequately to reduce polymerization shrinkage.

7. Finishing & Polishing

Occlusion was checked in static and dynamic movements.
Finishing burs, discs, and polishing spirals were used to recreate natural occlusal anatomy.
Final gloss achieved with polishing paste for enamel-like luster.

8. Final Result

Restorations demonstrated proper occlusal contacts, anatomical morphology, and tight proximal contacts.
The outcome was functionally stable, esthetically pleasing, and biologically respectful.
